Summit Lathe machine - On or about April 9, 2018, the (Summit model 24X120B S/N 31852) lathe machine's chuck guard was not in use to protect the employees from coming in contact with rotating parts.      Instance b. Summit lathe machine - On or about April 9, 2018, the (Summit model 24X120B S/N 31852) lathe machines lead screw and driveshaft were not guarded to prevent employees from coming in contact with the rotating parts.      Instance c. Jet Drill Press - On or about April 9, 2018, an employee assigned to operate the (Jet model JDP-15M/MS) freestanding drill press was holding materials with gloved hands during the drilling procedures and was exposed to being pulled into the rotating drillbit while performing this task.
