Gramby Street, Manheim PA - The safety monitor was observed working where a safety monitoring system was in place thereby exposing employees to an approximate 35 foot fall hazard to the ground below, on or about April 23, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Main Roof; 123 E. Gramby Street, Manheim PA - Employees were observed conducting roofing operations on the main roof of the building and were exposed to an approximate 26 foot fall hazard to the ground below, on or about April 23, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
