29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:    a) Upper Roof of Area C; 123 E. Gramby Street, Manheim PA - The safety monitor was observed working where a safety monitoring system was in place thereby exposing employees to an approximate 35 foot fall hazard to the ground below, on or about April 23, 2018.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Gramby Street, Manheim PA - The employer did not prepare a written certification verifying that employees had been trained in hazards related to falls from elevation, on or about April 23, 2018. 29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) Main Roof; 123 E. Gramby Street, Manheim PA - Employees were observed conducting roofing operations on the main roof of the building and were exposed to an approximate 26 foot fall  hazard to the ground below, on or about April 23, 2018.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.    High Peak Roofing and Painting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1234449, citation number 1, item number 1 and was affirmed as a final order on July 3, 2017, with respect to a workplace located at 150 Pitney Road, Lancaster, PA 17601.
