29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) Main Entrance, A-wing East - Both safety monitors were observed working and not monitoring employees performing roofing activities, thereby exposing employees to a 27 foot high fall hazard to the ground below, on or about August 20, 2020. b) Main Entrance, A-wing East - Both safety monitors were observed working and not monitoring employees performing roofing activities, thereby exposing employees to a 7'6" high fall hazard to a lower level roof, on or about August 20, 2020. High Peak Roofing & Painting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(h)(1)(v), which was contained in OSHA inspection number 1310716, citation number 1, item number 1a and was affirmed as a final order on June 11, 2018, with respect to a workplace located at 123 E. Gramby Street, Manheim, PA 17545. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section (fall protection training) by preparing a written certification record which contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: a) Main Entrance, A-wing East - The employer did not prepare a written certification verifying that employees had been trained in hazards related to falls from elevation, on or about August 20, 2020. High Peak Roofing & Painting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1310716, citation number 1, item number 1b and was affirmed as a final order on June 11, 2018, with respect to a workplace located at 123 E.
