29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:           a) On or about April 25, 2018, employees were exposed to the hazard of falling approximately 24 feet while working from ladder jack scaffolding without the benefit of fall protection.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.452(k)(1): Ladder jack scaffold platforms exceeded a height of 20 feet (6.1 m):              a) On or about April 25, 2018, employees were exposed to the hazard of falling approximately 24 feet during the performance of work from a ladder jack scaffold that exceeded the 20 foot maximum permissible height for ladder jack scaffolding.           In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):              a) On or about April 25, 2018, employees were exposed to the hazard of falling while performing roofing work without fall protection from pitched roof surfaces having a ground to eave height of approximately 15 feet.          No abatement certification or documentation required for this violation.
