(b) On or about April 25, 2018 and at times prior to that date, for the employees exposed to amputation and/or struck-by hazards where a Delta drill press, model number DP400, serial number 011549W2043 was operated in the carpentry shop located at 5000 Demers Avenue, Grand Forks, North Dakota.
