29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used.    a) On or about April 26, 2018, at the above referenced worksite, an employee working on the Administration Building was engaged in bricklaying activities from a scaffold platform greater than 2 feet above the point of access was not provided ladder access. Employees were thereby not provided an adequate means of access / egress.     b) On or about April 26, 2018, at the above referenced worksite, employees working on an outbuilding in the northwest corner of the project were engaged in dismantling scaffolding. The employees were not provided ladder access to each level of the scaffold that was greater than 2' above the next lower level. Employees were thereby not provided an adequate means of access / egress.     There is no abatement certification of documentation required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.    a) On or about April 26, 2018, at the above referenced worksite, employees working on an outbuilding at the northwest corner of the project were engaged in scaffold dismantling activities where the ladder access did not extend at least 3' above the surface being mounted. Employees were thereby exposed to fall hazards of approximately 14 feet, when using the ladder as a means of access/egress.     There is no abatement certification of documentation required for this item.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift.     a) On or about April 26, 2018, at the above referenced worksite, an employee working on the Administration Building was engaged in bricklaying activities from a scaffold where a pre-operation inspection was not conducted by a competent person. At the time of OSHA inspection, the scaffold had deficiencies in guard-railing and ladder access. Employees were thereby exposed to fall hazards of approximately 14 feet.     There is no abatement certification of documentation required for this item.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.     a) On or about April 26, 2018, at the above referenced worksite, an employee working on the Administration Building was engaged in bricklaying activities from a scaffold more than 10 feet in height. The employer failed to provide fall protection to protect the employee from falls, thereby exposing the employee to fall hazards of approximately 14 feet.      There is no abatement certification of documentation required for this item.
