29 CFR 1910.178(n)(1): Traffic regulations including authorized plant speed limits were not observed: On or about April 20, 2018, in the Live Receiving back dock area, employee operating Toyota forklift, s/n 10781, Model 8FD35U, traveled in excess of the authorized plant walking speed limit while transporting an elevated empty chicken cage. Employees were exposed to struck and crush-by hazards.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: On or about April 20, 2018, in the Live Receiving back dock area, employee operating Toyota forklift, s/n 10781, Model 8FD35U, traveled with an empty chicken cage elevated on the forks of the forklift impeding his view. This condition exposed employee to struck and crush-by hazards.
