B) Yard Area, on or about April 27, 2018, employees operated a Kalmar Ottawa T2 powered industrial tractor #215 that had only one operating head light during hours of darkness and a non-functioning horn, exposing employees to a struck by hazard.

29 CFR 1910.176(b): Storage of material created a hazard: A) Warehouse Aisle 7, storage rack area 38B, on or about April 24, 2018, loose boxes were improperly stacked on the second level of the rack system, exposing employees to a struck by hazard. B) Warehouse Aisle 15, storage rack area 11C, on or about April 24, 2018, a pallet located on the third level of the racking system had partially fallen down into the rack, exposing employees to a struck-by hazard. C) Warehouse Aisle 29, storage rack area 203D, on or about April 24, 2018, a pallet with boxes located on the fourth level of the rack system was leading outward over the aisle, exposing employees to a struck-by hazard. D) Warehouse Aisle 30, storage rack area 233E, on or about April 24, 2018, a pallet located on the fifth level of the rack system with boxes on it had partially fallen down into the rack system, exposing employees to a struck-by hazard. E) Warehouse Aisle 30, storage rack area 242B, on or about April 24, 2018, loose boxes on the second level of the rack system were improperly stacked, exposing employees to a struck by hazard. F) Warehouse Aisle 40, storage rack area 54E, on or about April 24, 2018, the bottom box on a pallet located on the fifth level of the rack system was crushed causing the pallet of boxes to lean outward over the aisle, exposing employees to a struck-by hazard. G G) Warehouse Aisle 40, storage rack area 82E, on or about April 24, 2018, a pallet of boxes located on the fifth level of the rack system The Dollar Tree Distribution, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 176(b), which was contained in OSHA inspection number 1019223, citation number 1, item number 1 and was affirmed as a final order on July 16, 2015, with respect to a workplace located at 300 Dollar Tree Lane, Joliet, IL. The Dollar Tree Distribution, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 176(b), which was contained in OSHA inspection number 966521, citation number 1, item number 1 and was affirmed as a final order on May 27, 2014, with respect to a workplace located at 99 international Drive, Windsor, CT.
