29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in �� 1926.502: Instance 1.) 29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded. On or about April 25, 2018, employees were installing metal trusses up to approximately 20 feet high with the lifeline wrapped around the truss without protection, exposing employees to a fall hazard. Instance 2.) 29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee can neither free fall more than 6 feet (1.8 m) nor contact any lower level. On or about April 25, 2018, employees were installing metal trusses up to approximately 20 feet high without ensuring the fall protection was rigged properly, exposing employees to a fall hazard.

The employer did not institute a training program and ensure employee participation in the program. On or about April 25, 2018, employees were installing metal trusses up to approximately 20 feet high without being trained in fall protection, exposing employees to a fall hazard.
