OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazard of being struck by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event the pipe ruptured:  a) On or about May 7, 2018, in the fabrication/production area, PVC plastic piping was used in exposed above ground areas to transport compressed air.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At the jobsite, employees were exposed to an approximate 16'-11" fall hazard, in that, fall protection was not used while removing an awning on a 4:12 pitch metal roof, on or about April 20, 2018.  b) At the jobsite, employees were exposed to an approximate 16'-11" fall hazard, in that, fall protection was not used while installing an awning on a 4:12 pitch metal roof, on or about April 28, 2018.  c) At the jobsite, employees were exposed to an approximate 23'-6" fall hazard, in that, fall protection was not used while removing an awning on a 4:12 pitch metal roof, on or about April 20, 2018.  d) At the jobsite, employees were exposed to an approximate 23'-6" fall hazard, in that, fall protection was not used while installing an awning on a 4:12 pitch metal roof, on or about April 28, 2018.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) At the jobsite, an employee was exposed to a fall hazard while carrying 4.5 feet wide x 2.5 feet height x 2.5 feet projection awnings down the ladder, on or about April 20, 2018.  b) At the jobsite, an employee was exposed to a fall hazard while carrying 4.5 feet wide x 2.5 feet height x 2.5 feet projection awnings up the ladder, on or about April 28, 2018.
