On or about May 01, 2018, located at 7484 Paradisio Drive, Lot 7872, Galena, Ohio, employees were installing stone façade on a residential home from a fabricated framed scaffold. The employer failed to initiate and maintain safety and health programs related to the work being performed, thereby exposing the employees the hazards associated with exterior stone work.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    a. On or about May 01, 2018, located at 7484 Paradisio Drive, Lot 7872, Galena, Ohio, employees applied masonry cement to wire mesh while under a fabricated frame scaffold that had no toe boards. Not all employees were wearing hardhats while performing the work under the scaffolding, thereby exposing employees to a struck-by hazard.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. When the falling objects are too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects away from the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling.    a. On or about May 01, 2018, located at 7484 Paradisio Drive, Lot 7872, Galena, Ohio, employees were on a fabricated framed scaffold working platform installing flashing while the scaffold did not have toe boards, thereby exposing employees to a struck-by hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.   a. On or about May 01, 2018, located at 7484 Paradisio Drive, Lot 7872, Galena, Ohio, employees performed exterior masonry work from a fabricated framed scaffold without adequate fall protection on the front of the scaffolding. The employer failed to install a midrail to protect employees from a fall hazard of approximately 12-1/2 feet.   b. On or about May 01, 2018, located at 7484 Paradisio Drive, Lot 7872, Galena, Ohio, employees performed exterior masonry work from a fabricated framed scaffold without adequate fall protection on the side of the scaffolding. The employer failed to install a guardrail system at the end bracket on the left side of the scaffolding when facing the residential home, thereby exposing the employees to a fall hazard of approximately 12-1/2 feet.
