29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 4/27/18, in Commercial Tenant # 4 Area , on construction site at 2178 Seneca Street, Buffalo,NY: A DeWalt Sawzall was connected to 120 volt-ac power via a series of extension cords. One of the extension cords in the series was a short 3-way extension cord, that lacked a ground prong on its plug. This interrupted the path to ground to the sawzall. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a) On or about 4/27/18, in Commercial Tenant # 4 Area , on construction site at 2178 Seneca Street, Buffalo,NY: A DeWalt Sawzall was connected to 120 volt-ac power via a series of extension cords. The outer sheath of the last cord in the series was a long cord that had many worn, damaged , or frayed areas, exposing the hot, neutral, & grounding conductors. NO ABATEMENT CERTIFICATION REQUIRED
