29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about 05/02/2018, United Fulfillment Solutions, Inc., located at 431 Bussen Underground Road, St. Louis, reported an in-patient hospitalization of one their employees. The employee was injured and hospitalized on April 26, 2018 and the employer reported to OSHA on April 30, 2018. United Fulfillment Solutions, Inc. did not report the in-patient hospitalization within twenty-four (24) hour timeframe.
