29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Colorado Roofing Products, LLC dba CeDUR located at 3590 Himalaya Rd. Aurora, CO 80011: On and preceding May 2nd, 2018, the employer did not ensure that procedures were developed & documented for the control of potentially hazardous energy when employees were engaged in cleaning and maintenance operations within the Pour Robot (ABB) and Pick Robot (ABB) cages. The employees enter the cages that enclose the robotic arms at least two times daily to perform cleaning and nozzle maintenance using only an interlocked gate to disable the equipment.  This exposes employees to struck-by hazards.

Aurora, CO 80011: On and preceding May 2nd, 2018, the employer did not conduct an annual or more frequent inspection of the energy control procedures for the control of potentially hazardous energy. The employees conduct regular maintenance on machinery and enter cages that enclose the robotic arms at least two times daily to perform cleaning and nozzle maintenance using only an interlocked gate to disable the equipment.  This exposes employees to struck-by hazards.

Aurora, CO 80011: On and preceding May 2nd, 2018, the employer did not ensure that the point of operation of a Rigid BS 14002 Vertical Band Saw was guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycles.  QA and sales employees use the Rigid Vertical band saw to cut samples for QA testing and presentation to customers.  The band saw has an adjustable guard installed which is left in the full up position which leaves 2-5" of exposed blade when making cuts. This exposes employees to caught-in hazards.
