29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces were maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  (a) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer did not ensure the walking surfaces in the production area were free of hazards. The production floors were slick; several employees were observed slipping while walking. This condition exposed employees to slip hazards.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer's lockout/tagout program was not effectively implemented to ensure employee safety during assigned servicing and maintenance tasks. The employer did not ensure that machinery was properly isolated and rendered inoperative before any employee performed any servicing or maintenance on the equipment. Operations employees were not required to lock and tag out equipment when performing servicing and maintenance tasks that fell under the lockout/tagout standard, nor did they receive authorized employee training. This condition exposed employees to amputation hazards and hazardous energy.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer did not ensure that machine guarding was provided on the KM line to protect flash trimming, sorting, and bundling employees from ingoing nip points on the conveyor belt. Points on the conveyor with inadequate guarding included but were not limited to:    -Tail pulley near sorting/bundling station    -Head pulley near flash trimming station    -Tail pulley near flash trimming station This condition exposed employees to crushing and amputation hazards.   (b) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer did not ensure that machine guarding was provided to protect the KM booth operator from ingoing nip points on the tail pulley of the KM conveyor belt in the Pour Robot cage. This condition exposed employees to crushing and amputation hazards.  (c) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer did not ensure that machine guarding was provided around the KM robots and mold conveyor system. Employees could enter into the caged KM area through two unlocked, latched gates that were not equipped with interlocks. Employees regularly entered these gates to pass through the plant, exposing them to crushing and amputation hazards.

A power strip was connected to a flexible cord in the flash trimming area of the KM line. KM line employees handled the electrical equipment on a daily basis. This exposed employees to shock and burn hazards.

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  (a) Colorado Roofing Products, LLC at 3590 Himalaya Road in Aurora, CO: On or before August 25, 2021, the employer did not ensure employees were safe while using flexible cords. An extension cord in the flash trimming area of the KM line, connected to a power strip used to power a standing fan, was damaged, with insulation that had partially pulled away from the plug, exposing electrical wires. This exposed employees to shock and burn hazards.

Maintenance employees regularly performed servicing and maintenance on equipment that required the use of lockout/tagout. This condition exposed employees to amputation hazards and hazardous energy.  Colorado Roofing Products, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1313212, Citation 1 Item 1b, and was affirmed as a final order on July 13, 2018, with respect to a workplace located at 3590 Himalaya Road, Aurora, CO 80011.
