(a) Pier at Becker Trucking - Floating Clubhouse Construction Site - On or about May 3rd, 2018 and at times prior thereto the employer exposed employees to drowning while transiting from the barges into the floating clubhouse construction project on the east and northside entrances. Steelhead Marine, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.605(b)(2), which was contained in OSHA inspection number 1286963, citation number 1, item number 4 and was affirmed as a final order on 3/13/2018 with respect to a workplace located at 5949 N. Basin Ave.Portland, OR. 97217

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50-volts or more were not guarded against accidental contact. (a) Steelhead Marine Inc. - Pier at Becker Trucking - Floating Home Construction Site - On or about May 3rd, 2018 and at times prior thereto, employees were exposed to an electrical shock of 240 volts when resetting breakers in an electrical distribution box that supplied power to the floating home construction project. Steelhead Marine, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.403(i)(2)(i), which was contained in OSHA inspection number 1286963, citation number 1, item number 2 and was affirmed as a final order on 3/13/2018, with respect to a workplace located at 5949 N. Basin Ave. Portland, Or. 97217.
