29 CFR 1926.605(d)(2): The employer is not ensuring that there is in the vicinity of each barge in use at least one U.S. Coast Guard-approved 30-inch lifering with not less than 90 feet of line attached:  (a) Workers building a boat house at Steelhead Marine are exposed to drowning in that the employer is not ensuring that at least one life ring was available.  This condition was last observed on 06/07/2019.  Notes:  1)  The employer shall ensure that a US Coast Guard approved 30 inch life ring, with at least 90 feet of line attached is readily available at accessible points at each waterside work area.  2)  Pursuant 29 CFR1903.19, the employer must provide documentation that this violation has been abated within 15 workdays of the date this citation is received.

This condition was last observed on 06/07/2019.  Notes:  1)  The employer shall ensure that a fixed or portable ladder is available near every vessel being worked.  2)  Pursuant 29 CFR 1903.19, the employer must provide documentation that this violation has been abated within 15 workdays of the date this citation is received.

29 CFR 1926.605(d)(3): Employees walking or working on the unguarded decks of barges are not being protected with U.S. Coast Guard-approved work vests or buoyant vests:  (a) Workers building a boat house at Steelhead Marine are exposed to drowning in that the employer is not ensuring that each worker wore a PFD.  This condition was last observed on 06/07/2019.  Notes:  1)  The employer shall ensure that employees are protected by personal flotation devices.  2)  Pursuant 29 CFR 1903.19, the employer must provide documentation that this violation has been abated within 15 workdays of the date this citation is received.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level is not being protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Workers who are demolishing a structure at Steelhead Marine are exposed to falls from approximately 35 feet without guardrail systems, safety net systems, or personal fall arrest systems.  This condition was last observed on 06/07/2019.  Notes:  1) The employer shall ensure that each worker is protected from falls when working at heights greater than 6 feet above the lower surface.  2)  Pursuant 29 CFR 1903.19, the employer must provide documentation that this violation has been abated within 15 workdays of the date this citation is received.

29 CFR 1926.605(b)(2): Employees are not provided safe access (i.e., ramp or walkway) to or from the wharf, float, or barge:  (a) Workers building a boat house at Steelhead Marine are exposed to drowning in that the employer is not ensuring that safe access is available, in that the access route leading to the floating home under construction passed across an unsafe bridge which consisted of 3 pieces of weathered plywood with no hand rails crossing the water below.  This condition was last observed on 06/07/2019.    (b) Workers building a boat house at Steelhead Marine are exposed to drowning in that the employer is not ensuring that safe access is available, in that the access route leading to a barge used for storage of supplies tools and materials passed across an unsafe bridge across the water below,  which consisted of a 2"x10" plank with no rails.  This condition was last observed on 06/07/2019.  Steelhead Marine, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.605(b)(2), which was contained in OSHA inspection number 1313379, citation number 1, item number 1 and was affirmed as a final order on 9/25/2018 with respect to a workplace located at 5949 N. Basin Ave.Portland, OR. 97217  Steelhead Marine, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.605(b)(2), which was contained in OSHA inspection number 1286963, citation number 1, item number 4 and was affirmed as a final order on 3/13/2018 with respect to a workplace located at 5949 N. Basin Ave.Portland, OR. 97217  Notes:  1)  The employer shall ensure that safe walkways are provided for access to all work areas.  2)  Pursuant 29 CFR 1903.19, the employer must provide documentation that this violation has been abated within 15 workdays of the date this citation is received.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours work-related incidents resulting in in-patient hospitalization, amputation or the loss of an eye:  (a) On or about 06/05/2019 and at times prior thereto, a worker at Steelhead Marine fell into the water from height and sustained injuries which required hospitalization which the employer did not report to OSHA.  Note: The employer shall ensure that all reportable injuries, including hospitalization, amputation, or loss of an eye. are reported to OSHA within the statutory time frame.
