On or about May 2, 2018 the excavated material was not kept at least 2 feet from the edge of the excavation. This condition exposed employee(s) to cave-in hazards. On or about May 2, 2018 the employer did not ensure that a protective system was used while employee(s) worked in a trench with previously disturbed soil. This condition exposed employee(s) to cave-in hazards.
