29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.  a. On or about April 25, 2018, a maintenance employee performing activities covered by this part, failed to affix an energy isolating device as part of the employer's energy control procedure for the Deflasher machine. During the maintenance and servicing actiivities associated with diagnosis, switch removal and switch replacement, an energy isolating device, such as a lock, tag, or a locked or tagged hasp, was not affixed to the electrical disconnect for this equipment. Due to this condition, the maintenance employee had potential exposure to caught-in, struck-by and potential crushing hazards.   b. On or about April 25, 2018,  energy isolating devices or equipment were not utilized by a maintenance employee performing activities covered by this part to isolate and control stored mechanical energy associated with the "Table" component for the Deflasher machine during this covered activity. During the maintenance and servicing actiivities associated with diagnosis, switch removal and switch replacement, the employee was exposed to caught-in, struck-by and crushing hazards associated with the stored mechanical energy from the "Table" component.

These employees performed covered activities that involved potential exposure to caught-in hazards associated with an unexpected startup of the machine and stored mechanical energy when the Deflasher "table" was in the "up" position.
