In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: 1910.147(f)(1)(ii) Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; a. On or about May 6, 2022, the employer failed to remove employees from the machine or equipment area . This LOTO failure resulted in an maintenance employee being hospitalized due to a fractured leg while performing paddle replacement on the paddle conveyor in the paddle conveyor area.

On or about May 6, 2022, the employer failed to utilize a LOTO procedure which afforded the employees with a level of protection equivalent to that provided by the implementation of a lockout or tagout device. This LOTO failure resulted in an maintenance employee being hospitalized due to a fractured leg while performing paddle replacement on the paddle conveyor in the paddle conveyor area.
