29 CFR 1910.36(d)(1) as required by 1960.8(b): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  a) On or about May 8, 2018 at the facility located at 3059 North Morton Street in Franklin Indiana, the employer had placed exits signs on four doors. All of the doors were locked, exposing employees to fire hazards.

29 CFR 1910.37(a)(3)as required by 1960.8(b): Exit route(s) were not kept free and unobstructed:  a) On or about May 8, 2018, the rear exit near the furnace room was difficult to open due to wood rot. b) On or about May 8, 2018, the rear exit near the lunch room was difficult to open due to wood rot. c) On or about May 8, 2018, the rear exit near the copier room was difficult to open due to wood rot. d) On or about May 8, 2018, the rear exit behind the manager's office was difficult to open due to wood rot. The bottom of the door frame was knocked out when the door was opened.  Employees were exposed to fire related hazards.

(b) On or about May 8, 2018 in the furnace room, the furnace was blocked by 2, thirty gallon cans of shredded paper.

The USDA Farm Service Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.157(e)(2) as required by 29 CFR 1960.8 (b) which was contained in OSHA inspection number, 1155832 notice number 1, item number 3a and was affirmed as a final order on September 21, 2016, with respect to a workplace located at 900 4th Avenue, Ste. 3, Audobon, Iowa.
