29 CFR 1910.22(d)(2): Hazardous conditions on walking / working surfaces were not corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired: Control Center: A sub floor panel (24 X 24 inches) located in front of a bathroom door inside the Control Center Room was removed. An employee stepped out of the bath room and fell 18 inches in the hole. The employee sustained a sprain.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panel board enclosures in wet locations were not weatherproof: Control Center: A junction box with knockouts was fixed to the concrete floor and was not elevated to prevent water from coming inside the box. The junction box contained energized conductors and was placed in a location that had been flooded. Employees were exposed to electrical shock and burn injuries.
