29 CFR 1910.147(a)(2)(ii)(B): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into an area on a machine or piece of equipment's point of operation or where an associated danger zone existed during a machine operating cycle. At a facility located 1008 US-67, Alvarado, Texas: at the E palletizer machine, the employer did not ensure the lock-out/tag-out procedures were followed during cleaning of the machine sensor exposing the employee to amputation hazards.

At a facility located 1008 US-67, Alvarado, Texas: the employer did not ensure adequate or continued training for energy control purposes, function, recognition of energy sources, removal of energy and the use of the energy control program and procedures were utilized exposing employees to caught-in/amputation hazards.
