29 CFR 1910.38(c)(2): The emergency action plan did not include procedures for emergency evacuation, including type of evacuation and exit route assignments: On or about May 8, 2018, employees working in Room 2 are exposed to carbon dioxide from flash freezing equipment and the employer had not developed procedures on responding to emergencies pertaining to alarms and functions from carbon dioxide releases.

29 CFR 1910.145(f)(7): Warning tags were not implemented to warn employees of a hazard level between "Caution" or "Danger": On our about May 8, 2018, employees working in Room 2 were exposed to carbon dioxide gas from cryogenic freezing equipment and signs were not posted warning the employees that the cabinet freezer contained high levels of carbon dioxide gas.
