29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) At 3716 Airport Blvd Mobile, Alabama (Pylon Sign Area): On or about May 09, 2018, and at times prior; the employer allowed his employees not to wear hard hats to perform service and repair on an overhead pylon sign that was damaged from inclement weather and potentially going to fall approximately thirty (30) feet to the ground below exposing employees to struck-by hazards.

29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift: a) At 3716 Airport Blvd, Mobile, Alabama (Pylon Sign Area): On or about May 09, 2018, and at times prior; the employer allowed his employee to work from an aerial lift platform without being tied off to the approved "lanyard anchor" exposing him to a fall of approximately thirty (30) feet to the ground below. The Scott Electric Sign Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.67(c)(2)(v), which was contained in OSHA inspection number 1279620, citation number 1, item number 1 and was affirmed as a final order on January 03, 2018, with respect to a workplace located at 22149 Hwy 59 Advanced Auto Parts Robertsdale, Al 36567 USA.
