29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) CHS - Grainland located at 34661 CO Rd 53, Amherst, CO 80721: On and preceding 5/9/2018, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet or more above a lower level was protected from falling by a guardrail system, safety net system, or personal fall arrest system. Grain elevator employees worked on a 10' platform during truck grain-loading operations. The platform had guardrails around three of the four sides. The side used for access via a fixed ladder was open without a means for fall protection in place such as, but not limited to, a hinged gate or personal fall arrest system. This exposed employees to falls up to 10'.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Amherst Co-operative Elevator Inc. Located at 34661 CO Rd 53, Amherst, CO 80721: On and preceding 5/9/2018, the employer did not ensure that the main pulley which drives the cup belt for the bins was adequately guarded. The pulley presented a rotating part with protruding nuts/bolts that was not adequately guarded. The pulley also presented an ingoing nip point where the belt rotated around the pulley. The conveyor system was used to transport and distribute grain to the various grain bins. This condition exposed employees to amputation hazards caused by rotating parts and ingoing nip points.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft ends did not present a smooth edge and end, and projected more than one half the diameter of the shaft: a) CHS - Grainland located at 34661 CO Rd 53, Amherst, CO 80721: On and preceding 5/9/2018, the employer did not ensure that a shaft coming from the main roller/pulley on the tripper was guarded. The shaft projected 7" from the main pulley and measured approximately 2.5" in diameter. The shaft rotated at the same speed as the main pulley. The conveyor system was used to transport and distribute grain from the boot pit to the various grain bins. The exposed employees to caught-in hazards.

29 CFR 1910.219(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) CHS - Grainland located at 34661 CO Rd 53, Amherst, CO 80721: On and preceding 5/9/2018, the employer did not ensure that the pulleys used in the power transmission of the east belt of the open conveyor grain transportation system were adequately guarded. The motor pulley, drive belts, and associated drive pulley were not adequately guarded along the sides of the apparatus. The conveyor system was used to transport and distribute grain to the various grain bins. This condition exposed employees to amputation hazards caused by rotating parts and ingoing nip points.

29 CFR 1910.219(e)(1): 1910.219(e)(1)(i) Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to the requirements specified in 29 CFR1910.219 (m) and (o). a) CHS - Grainland located at 34661 CO Rd 53, Amherst, CO 80721: On and preceding 5/9/2018, the employer did not ensure that the power transmission belts, which goes from the motor to the gearbox driving the main pulley for the open conveyor grain transportation system, were adequately guarded. The belts were not adequately guarded along the sides of the moving belts. The conveyor system was used to transport and distribute grain from the boot pit to the various grain bins. This condition exposed employees to amputation hazards from rotating parts and ingoing nip points.
