29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Eldorado Stone, LLC, 181 Antrim Way, Greencastle, Pennsylvania - An employee suffered an amputation injury to the right thumb tip after getting the thumb pinched between a pallet and a pallet backstop on April 2, 2018. The employer failed to report this work-related amputation to OSHA within 24 hours. (The employer reported this amputation to OSHA on April 5, 2018.) Corrected during the inspection.
