This was most recently documented at 200 S 2nd St, Lincoln, Nebraska on May 5, 2018 when employees were exposed to in going nip points from an unguarded conveyor belt and roller.  On May 3, 2018, an employee made contact with an unguarded conveyor belt and roller which resulted in a finger tip amputation.
