One procedure was written to cover all equipment in the plant including but not limited to various makes and models of CNC milling centers, CNC lathe centers, and other machines.  Authorized personnel are instructed by the procedure to assess and identify the type and magnitude of energy that the machine or equipment to be secured, utilizes.  Specific procedural steps are not documented for specific machines or machine types [B].  The procedure instructs placement of locks and tags, but no instruction states what to attach them to, such as valves or switches, or where to find the components to isolate and lock [C].  Authorized personnel is instructed to make sure the machine being locked out is secured, but specific actions required to test that an isolated machine is secured, are not documented [D].
