29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about May 8, 2018, during the installation of roofing material, the employer did not instruct/train exposed employees to ensure the companys employees recognized unsafe conditions while setting up a TranzSporter Platform Hoist attached to a portable ladder that came into contact with an energized power line 21.8.5 feet above head.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about May 8, 2018 at the job site located at 7285 Meadow Lane, Parma, Ohio 44129; during the installation of roofing material on a residential roof, the employer did not ensure employees that were setting up a TranzSporter Platform Hoist (Model TPZ250) attached to a portable ladder (28 foot aluminum portable ladder ) were of clear and safe distance from the energized power line (7620 volts) above head (21.8.5 feet) at their work location when the ladder became energized shocking the employees causing injury.
