29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 22, 2023, during the installation of roofing material at a job site located at 13053 Cherokee TR, Middleburg Heights Ohio 44130; the employer did not ensure employees working on a roof of a residential home were protected from a fall of approximately 9.5 feet by utilizing a fall protection system.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 22, 2023, at a work-site located next to 13053 Cherokee TR, Middleburg Heights Ohio 44130; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects from working surfaces of heights approximately 9.5 feet.  J.D. Roofing and Exteriors INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.100(a) Duty to have head protection), which was contained in OSHA inspection number 1506459, citation number 01, item number 01 and was affirmed as a final order on 01/28/2021, with respect to a workplace located at 941 East Meadow Lane Blvd, Seven Hills Ohio 44131.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 22, 2023 during the installation of roofing materials at a job site located at 13053 Cherokee TR, Middleburg Heights, Ohio 44130; the employer did not ensure that portable ladders extended 3 feet above the upper landing surface exposing employees to fall hazards. The owner/foreman was on site and working alongside of the exposed employees.  J.D. Roofing and Exteriors INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Ladders), which was contained in OSHA inspection number 1506459, citation number 03, item number 01 and was affirmed as a final order on 01/28/2021, with respect to a workplace located at 941 East Meadow Lane Blvd, Seven Hills Ohio 44131.
