At the site located at 350 Anchor Rd, Casselberry, Florida, employees were exposed to a 16  foot fall hazard, in that, while working on a roof 300 feet wide by 300 feet long employees were handling roofing material and doing repair work right at the edge of the roof without fall protection, on or about 05/08/2018.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a. At the site located at 350 Anchor Rd, Casselberry, Florida, employees exposed to fall hazards, were not provided training and education relating to fall protection, hazard recognition, procedures to be followed to minimize these hazards and the proper use and limitations of fall protection equipment and fall protection systems:  1. During roofing work employees were exposed to fall hazards while on the roof outside the control access area; 2. A monitor system was used on a roof over 50 feet in width; 3. While using a fall arrest system a fall arrest distance greater than 6 feet was allowed; 4. The monitor did not stay close to where the work was done and at times he was about 100 feet away from the workers; 5. While handling, removing old panels and setting up a safe walkway on the roof, employees were exposed to a fall hazard.  On or about 05/08/2018.

On or about 10:45 a.m. on 04/24/2018, at 350 Anchor Rd, Casselberry, Florida, an employee suffered a work related injury and was hospitalized. The employer was aware of the injury and hospitalization on the same day of the incident. The employer notified OSHA on 05/04/2018 of the in-patient hospitalization.
