29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. On or about 5/4/2018 the employer did not ensure that aisle ways were kept clear in order for Powered Industrial Vehicle operators to be able to maneuver while conducting material handing operations.

29 CFR 1910.178(n)(3): Other trucks traveling in the same direction at intersections, blind spots, or other dangerous locations shall not be passed. On or about 5/4/2018 three employees were operating powered industrial trucks in a congested aisle, the third PIV operator was traveling South, the first and second PIV operators were traveling North; after the second PIV operator passed the first, a collision occurred resulting in an injury.
