29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:     On or about May 15, 2018, during the installation of framing material on a new construction residential structure, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from a height of approximately 12.2 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:       On or about May 15, 2018, during the installation of framing material at a job site located at 13220 Trenton Tr., Middleburg Heights, Ohio 44130 the employer did not ensure employees working on the second floor of the structure were protected from a fall of approximately 12.6 feet by utilizing a fall protection system     a. Cloverhill Construction LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1050577, citation number 01, item number 01 and was affirmed as a final order on May 27, 2015 with respect to a workplace located at 4975 Bridgewater Ln, Brunswick , OH 44121.     b. Cloverhill Construction LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1064730, citation number 01, item number 01 and was affirmed as a final order on June 23, 2015 with respect to a workplace located at 12504 Saddlebrook, Strongsville, OH 44136.
