29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about May 15, 2018, the employer failed to ensure that a means of egress was available for employees tasked with working inside a trench to tie-in a fire hydrant. The trench was 6.5 feet deep and 6 feet wide with no cave-in protection present.

The trench was 6.5 feet deep and 6 feet wide without benching or sloping present.
