29 CFR 1910.305(a)(2)(ix): Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard: a) The stewarding chemical storage room: The unprotected light was mounted approximately 60 inches from the floor on the sloped edge under the stairs. Employees were exposed to laceration hazards. b) The 3rd floor housekeeping and electrical storage room: The unprotected lights were mounted approximately 81 inches from the floor and items were stored approximately 12 inches from the light bulbs. Employees were exposed to laceration hazards. c) The 4th floor housekeeping and electrical storage room: The unprotected lights were mounted approximately 81 inches from the floor and items were stored approximately 4 inches from the light bulbs. Employees were exposed to laceration hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Training was not provided to employees who worked with corrosives, and other chemical hazards when they were assigned to work with the chemicals. Employees were exposed to chemical hazards.
