OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 SECTION (5)(a)(1): The employer did not furnish employment and  a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:    a)  Printing Press 238, on or about 4/18/18:  CM 1/2-Ton Hoist had not been inspected on a frequent and periodic basis.    b)  Printing Press 238, on or about 4/18/18:  Under the hook lifting devices, i.e. spreader bar and lifting components,  had not been inspected on a frequent and periodic basis. While loading a 492 pound anilox roller  into the press, an employee sustained a serious hand contusion when the lifting eye bolt unexpectedly broke.  The lifting eye bolt was rated to lift 440 pounds.      c)  Printing Press 238, on or about 4/18/18:  Monorail had not been inspected on a frequent and periodic basis.    Among other methods, one feasible method of abatement is to:        1)  Inspect the hoist on a frequent and periodic basis for wear, deterioration and malfunction as per ASME B30.16-2017, Section 16-2.1.        2)  Inspect the under the hook lifting devices on a frequent and periodic basis for wear, deterioration and malfunction as per ASME B30.20-2013, Section 20-1.3.1.        3)  Inspect the monorail on a frequent and periodic basis for wear, deterioration and malfunction as per ASME B30.11-2010        4)  Ensure not to exceed the rated load of the lifting device nor the capacity of the hoisting equipment by the combined weight of the load, the lifting device and rigging.
