29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about 5/9/18, the employer failed ensure the following steps were taken when employees were performing maintenance on the stuck plungers on the NS2 Assembly, Line A, Sonitek T500 Thermal Presses: 2 A, 2 B, 4 A, 4 B, 7, 10 A, 10 B, and 10 C.,  (Note: all steps are to be done by an authorized employee(s)):     1) Prepare for shutdown,  2) Shutdown machine or equipment,  3) Machine/equipment isolation,  4) Apply lockout device(s),  5) Relieve potentially hazardous energy sources, and    6) Verify energy isolation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 5/9/2018, the employer failed to ensure that adequate machine guarding methods were utilized on machinery including, but not limited to the following:    a) NS2 Assembly, Line A, Sonitek T500 Thermal Presses: 2 A, 2 B, 4 A, 4 B, 7, 10 A, 10 B, and 10 C. The thermal presses were equipped with barrier guards that were attached with plastic rotatable nuts, allowing for easy removal by hand, and     b) NS2 Assembly, Line A, Sonitek T500 Thermal Press 10 A, had an opening between the barrier guard and the conveyor track of about 2.5". The width of the barrier guard was about 10 & 1/2",  and the distance from guard to the point of operation was about 1", and      c) NS2 Assembly, Line A, station #6 had a guard that was affixed with a hinge, and a rotatable lever secured with a flat head screw.    UNICONTROL, INC. WASPREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVELANT STANDARD, 29 CFR 1910.212(a)(3)(ii), WHICH WAS CONTAINED IN OSHA INSPECTION #1193607, CITATION NUMBER 01, INTEM NUMBER 01, AND WAS AFFIRMED AS A FINAL ORDER ON JANRUARY 19, 2017 WITH RESPECT TO A WORKPLACE LOCATED AT 1111 BROOKPARK ROAD, CLEVELAND, OHIO 44109.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting was not provided to all employees upon initial employment, and at least annually thereafter:    Note:  Applies where all employees are expected to fight fires.      On or about 5/17/18, the employer did not have an educational program to train employees on the principles of fire extinguisher use in the workplace and the hazards involved with incipient stage firefighting.      Note:  Employer may exercise one of the following options:      1) Provide portable fire extinguishers as specified in 29 CFR 1910.157(c)(1), meet the  distribution requirements per 29 CFR 1910.157(d) (75' to the nearest extinguisher for Class A   fires and 50" to the nearest unit for Class B fires) and provide training and education for all  employees in the use of such equipment and the hazards associated with incipient stage firefighting as required in 29 CFR 1910.157(g). The other sections of 29 CFR 1910.157 also  apply.      2) Provide extinguishers as specified in 29 CFR 1910.157(c)(1), designate certain employees  to be the only employees authorized to use such equipment and require all other employees to  immediately evacuate upon the sounding of the fire alarm, establish this in writing in an   emergency action plan as outlined in 29 CFR 1910.38(c) and train/educate designated employees   per 1910.157(g). Utilizing this option exempts the employer from distribution requirements of   29 CFR 1910.157(d).      3) Establish a written fire safety policy which requires the immediate and total evacuation of  employees from the workplace upon the sounding of a fire alarm signal. This policy includes  an emergency action plan and a fire prevention plan as specified in 29 CFR 1910.38 (a)  and (b). Utilizing this option exempts the employer from requirements of 29 CFR 1910.157 to  include the need for providing portable fire equipment unless a specific standard requires that  a portable fire extinguisher be provided (such as those requirements in 29 CFR 1910.106/107  relating to use/storage of flammable/combustible liquids). If extinguishers are provided but not  intended for employee use, 29 CFR 1910.157(a) and (f) are still applicable (inspection,  maintenance and testing of equipment).
