29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:    a. On or about 05/17/2018, at 4921-4935 Windermere Ave. - an employee was exposed to an electric contact hazard while using a flexible cord that was spliced together. The two flexible cords were connected using electric black tape due to a damaged cord plug.

Employees installing roofing tiles at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on May 17, 2018, when employees installing roofing tiles on the 6:12 pitch roof of a house at 4921-4935 Windermere Ave. in Kissimmee, Florida, were exposed to the hazard of a 19 foot 2 inch fall to the ground in that they did not have roof anchor points to attach personal fall arrest gear.      Crown Roofing LLC was cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(11), which was contained in OSHA Inspection number 1136266, Citation Number 01, Item Number 001 and was affirmed as a final order on 09/12/2016, with respect to a workplace located at Courtyard Esterro Apartments Building 17, Corkscrew Commons Drive, Estero, FL 33928.      Crown Roofing LLC was cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection number 1115276, Citation Number 01, Item Number 001 and was affirmed as a final order on 04/01/2016, with respect to a workplace located at 6622 Everton Court, Fort Myers, FL 33966.
