29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment.    On or about 2/8/2018, employees were required to wear smocks with loose fitting sleeves while operating the Ham Boning Area, Marel Skinner Machine. The employer's personal protective equipment (PPE) assessment failed to determine smocks with loose fitting sleeves exposed employees to caught-in hazards presented by the rotating wheel and cutting blade of the machine.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  On or before February 8, 2018, the employer did not ensure employees utilized the procedures for the safe application, usage, and removal of potentially hazardous energy controls while servicing the Marel Skinner in the ham boning area.  Energy sources include mechanical and electric power.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   On or about February 8, 2018, the employer did not ensure employees implemented energy control procedures during service and maintenance activities, such as blade changes and cleaning of the Marel Skinner Machine, by affixing a lockout device to each energy isolating device [per the 1910.147(d)(4) requirements], and verifying the equipment energy was isolated and de-energized prior to starting work [(d)(6)].

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    On or about February 8, 2018, the employer did not ensure employees designated as authorized to perform service and/or maintenance activities on the Marel Skinner Machine, such as changing blades and cleaning the machine, were trained to recognize hazardous energy sources as well as the means necessary to isolate and control them.
