29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) New residential construction located on Lot #16, Roof: On or about 05/21/2018, and at times prior, the employer exposed employees to fall hazards of 19-feet, 1-inch to the lower level, in that adequate fall protection systems were not provided to employees installing 15# roofing felt. Employees wearing full body harnesses were not connected to life-lines.

29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: (a) New residential construction located on Lot #16, Roof: On or about 05/21/2018, and at times prior, the employer exposed employees to fall hazards of 11-feet, 1-inch to the lower level, in that employees installing roofing felt on the rear roof were using life-lines which were wrapped around metal truss gussets and not protected from rips or tearing during use.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: (a) New residential construction located on Lot #16, Roof: On or about 05/21/2018, and at times prior, the employer exposed employees to fall hazards of 11-feet, 1-inch to 19-feet, 1-inch, in that the employer failed to train employees correct procedures to erect, maintain and inspect personal fall protection systems. Anchors and life-lines were not used according to the manufacturer's specifications.
