29 CFR 1910.110(b)(11)(v)(f): Vaporizer-burners were not equipped with automatic safety devices to shut off the flow of gas to the main burner and pilot in the event the pilot is extinguished: On or about May 9, 2018, during the Refractory Dry Out project at the Dynegy Midwest Generation Plant, a vaporizer-burner was not equipped with a functional automatic safety reset valve. The device did not shut off the flow of gas when the pilot was extinguished.
