29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) New residential construction located at 206 Blue Point Parkway, Fayetteville, GA, Front roof: On or about 05/23/2018, and at times prior, the employer exposed employees to falls of 10-feet, 4-inches while working on a 8/12 pitched roof, in that a fall protection system was not provided to employees engaged in roofing activities.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 206 Blue Point Parkway, Fayetteville, GA 39215, Garage (South) side: On or about 05/23/2018, and at times prior, the employer exposed employees to falls of up to 11-feet, 2-inches to the lower level, in that the ladder used to access the roof and ground levels was not extended at least 3-feet past the eave or secured to prevent movement during use.
