On or about May 22, 2018, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about May 22, 2018, at the above addressed jobsite, employees performing residential roofing operations on a roof ten (10) feet eight (8) inches above the ground were not protected from falls, thereby exposing employees to fall hazards. Domingo Garcia dba Domingo Construction Incorporated was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1242280, citation number 1, item number 1a and was affirmed as a final order on December 5, 2017, with respect to a workplace located at 19524 Brookridge Drive, Tinley Park, IL.
