Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. NR Lee Restoration, Ltd. at the worksite located at 301 N. Main St. in Lima, Ohio: On or about May 23, 2018, the employer did not assure employees were protected from falls while performing tasks associated with roofing. Employees performing roofing operations on a commercial roof were exposed to falls of approximately 20 feet and up to approximately 50 feet on the unprotected edges of the roof.

Main St. in Lima, Ohio: On or about May 23, 2018, the employer did not ensure that the employer fabricated horizontal lifeline was designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system. Employees utilizing the horizontal lifeline during low-sloped roofing operations were exposed to potential falls in that the employers design and installation of the system did not incorporate engineering data, identifying the specifications of installation and limitations of use.

Main St. in Lima, Ohio: On or about May 23, 2018, the employer did not ensure that fall protection anchorage was installed with the required fasteners to support at least 5,000 pounds. Employees that utilized the Guardian Temper Anchors while performing roofing work were exposed to fall hazards of up to approximately 50 feet in that the D-ring anchors were not properly installed.
