29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about May 23, 2018, employees were exposed to a 13.5 foot fall while performing roof installation operations on a single-family home.  The employer failed to ensure employees were protected from falling through skylights greater than 6 feet above lower levels by personal fall arrest systems, covers, or guardrail systems erected around such holes.  No abatement certification or documentation is required of this item.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about May 23, 2018, employees performing roof installation operations on a single-family property were exposed to a 13.5 foot fall when the employer failed to ensure that fall protection was utilized.   No abatement certification or documentation is required of this item.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about May 23, 2017, employees performing roof installation operations on a residential property were exposed to a fall of 13.5 feet while working without fall protection.  The employer failed to ensure the employees were adequately trained to recognize and minimize the hazards associated with falls.  Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).

On or about May 23, 2018, employees were exposed to a 13.5 foot fall hazard when the employer failed to ensure the ladder side rails extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.
