29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) Stretching, 908 East Elm Street, Scranton, PA - An employee was permitted to voluntarily wear a half-mask elastomeric respirator during screen stretching operations when applying glue to a wooden screen and was not provided with training that contained the information in Appendix D of this standard, as observed on or about May 24, 2018. b) Wood Shop, 908 East Elm Street, Scranton, PA - Employees were permitted to voluntarily wear 3M Particulate Respirator 8200/07023 (AAD), N95 dust masks when cutting wood and were not provided with training that contained the information in Appendix D of this standard, as observed on or about May 24, 2018. c) Grinding Room, 908 East Elm Street, Scranton, PA - Employees were permitted to voluntarily wear 3M Particulate Respirator 8200/07023 (AAD), N95 dust masks when grinding aluminum and were not provided with training that contained the information in Appendix D of this standard, as observed on or about May 24, 2018. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: a) ) Stretching, 908 East Elm Street, Scranton, PA - An employee was permitted to voluntarily wear a half-mask elastomeric respirator during screen stretching operations when using glue and was not provided with a respirator medical evaluation and instruction on the cleaning, storage, and maintenance of the respirator, as observed on or May 24, 2018. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
