29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about May 24, 2018, employees were exposed to a struck by hazard while installing siding materials on a residential home located at 5181 Duxbury, Akron, Ohio 44321. Employees were working on the ground, underneath other employees working on the roof, and the employer did not ensure employees wore protective helmets.

On or about May 24, 2018, employees were exposed to a fall of approximately 12 feet while installing siding materials on a residential home located at 5181 Duxbury, Akron, Ohio 44321. The employer did not ensure employees working on all areas of the roof used fall protection systems/devices.
