29 CFR 1910.37(b) (4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  Southern Tire Mart, Covington, GA (Throughout Facility) - On or about May 29, 2018, and times prior to, there were several designated, marked, and identified emergency exit routes and doors located throughout the facility which were hidden by numerous piles of tires and machines.  These products blocked the view of the exit routes, signs and doors.  There were no directional exit markings indicating the way to travel, exposing employees to an entrapment hazard. 29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  Southern Tire Mart, Covington, GA (Throughout the facility) - On or about 5/29/2018, and times prior to, tires were stacked without organization or being secured which created a potential hazard of tires sliding and falling on employees due to constant forklift traffic on employees. Forklift drivers were hitting the tires while driving.  The tires were blocking exits and aiselways which created a hazard for other forklifts and pedestrians walking in the area. 29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    Southern Tire Mart, Covington, GA - An employee buffing tires at Buffer #2 was exposed to noise levels of 87.6 dBA which is in excess of the action level of 85 dBA. The exposure was derived from a sample taken over 436 minutes. Zero exposure was assumed for the unsampled period of 44 minutes. A hearing conservation program including monitoring, performing hearing test and providing noise training, was not implemented, on or about 06/29/18.
