29 CFR 1910.305(b)(3)(i): Pull and junction boxes used on systems rated over 600 volts, nominal, did not provide a complete enclosure for the contained conductors or cables:  On or about February 15, 2018, and in times prior thereto, the side panel covers were missing from adjoining Ampgard, 4160 Volt electrical starter cabinets, exposing workers to the hazard of an arc flash, located in the Cub 2 Building.

29 CFR 1910.335(a)(2)(ii): Protective shields, protective barriers, or insulating materials were not used to protect each employee from shock, burns, or other electrically related injuries while that employee is working near exposed energized parts which might be accidentally contacted or where dangerous electric heating or arcing might occur.   On or about February 15, 2018, and in times prior thereto, where the side panel covers were missing from adjoining Ampgard, 4160 Volt electrical starter cabinets, shields or barriers were not used to isolate the live parts of the adjoining panels while employees performed work near them, exposing workers to the hazard of an arc flash, located in the Cub 2 Building.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  On or about February 15, 2018, and in times prior thereto, electricians were working next to 4160 volt live parts that were not deenergized or insulated without using electrical arc flash protective equipment for face, hands and body.  The close proximity to live buss bars exposed the electricians to an arc flash hazard, located in the Cub 2 Building.

29 CFR 1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts:  On or about February 15, 2018, and in times prior thereto, electricians were using a 16 inch torque wrench that was not insulated within one inch of 4160 volts of electricity.  The close proximity of this wrench to live buss bars exposed the electricians to an arc flash hazard, located in the Cub 2 Building.
